5) Final trim size Flat
6) Final trim size folded if different than above
7) Stock weight required
8) Quantity as well as additionals required
9) Delivery location. (City, and province/state)
10) Required completion date
11) Any special handing or delivery instructions

*4) For the number of pages this is the number after the sheet is folded. Eg. if you take an 11" x 17" sheet and fold in half you have 4 pages.
